Residents living area codes 75074, 75442 and 75407, join the American Heart Association, CityBridge Urgent Care, and the Assistance Center of Collin County to participate in a health program to improve blood pressure.

Participants receive:

  • A blood pressure kit, including machine, weight scale, pillbox, cookbook, and blood pressure log
  • A free visit with a healthcare provider
  • Instructions on how to measure, manage, and log blood pressure readings
  • Monthly visits (virtually or in-person) with a health coach to receive health education
