Organizations participating in our Outpace CVD programs and partnered initiatives gain access to our data platform (aha.infosarioregistry.com). Here, they can utilize benchmarking reports to support their quality improvement activities and submit data towards achievement awards in Target: BP™, Check. Change. Control Cholesterol™, and Target: Type 2 DiabetesSM.

Step 1: New Participant Registration - Getting Access & Support

The American Heart Association’s Outpace CVD programs and initiatives offer our data platform and benchmarking reports as one tool in your quality improvement journey.

Step 2: Get Trained on the Platform

Before submitting for award achievement, familiarize yourself with the data platform. Review our helpful resources to learn how to gain access, setup your site details, submit data for awards and make the most of the available operational reports in your quality improvement journey.

Step 3: Benchmarking and Award Achievement - Submitting Aggregate Data

Ready to be recognized? Sites that would like to be considered for an achievement award are required to submit information for the previous calendar year in the data submission platform.

Recognition data submission opens on January 1. The deadline to submit 2024 data for recognition in 2025 is Friday, May 16, 2025 at 11:59 PM ET.

Step 4: Utilizing the Data Platform for Benchmarking

Beyond receiving achievement awards, submitting data includes self-attestation questions that allow you to identify where you may have gaps in standard best practices. Your annual data also populates reports that allow you to track year-over-year data and see how your organization compares to others in various benchmarking groups (including state, urban/rural, FQHC, etc.).

Step 5: Work with us on Quality Improvement

Outpace CVD offers a variety of approaches to support you along your quality improvement journey. Our consultation services allow for AHA to identify opportunities and solution for barriers using your self-reported data, as well as navigate our repositories for resources to fit your needs. In a self-guided approach, you are welcome to utilize our library of chronic disease resources, monitor your progress through data benchmarking and check-in with us as you see fit.
